Should this be a single, circular hole? This is the tube in the steering column that is used for gear selection.

Also for the column shift 3 spd, is the H pattern determined strictly by how the transmission falls into place, or is there some sort of h pattern guide in the column?

I wish I could give you a good answer...but it does look worn

The H pattern is determined by the shifter mechanism. The transmission is at play but basically it just needs a device to throw the shift levers.
