Here is the carrier I built last fall for El Choco Beasty, I based it off of the military HMMWV carrier.
Like your shoulder, I have a bad back and was not about to try and lift 150+ lbs. of 37" tire and rim in and out of the cargo area, it also takes up a huge amount of floor space.
I can unlock and lower or raise and stow the mounted tire in under 3 minutes using the ratchet.

It uses a 2 way ratcheting system to raise and lower the tire, a T-handle to hold the stowed carrier to the bumper and a nylon pad under the arm to ride on the top of the bumper. Aside from the T-handle, I also can apply downward pressure to the system. It does not move.

There are 2 swing out lock positions, 45* and 90*, a spring loaded pin holds the location.

The spare is extended down and touching the ground. This is a 35" that came with the set of rims that I bought for spares and trailer wheels, I just haven't need to mount a 37" spare yet (it doesn't leave the acreage). The four mounted are 37". My bumper top is at 36"

License plate mount is receiver hitched and removeable to get the spare on and off.
